Need Help on Movie Produciton

I am currently working on a movie in Arkansas..the script calls for a high school senior to "sign with" a major college to play a football. Can anyone get me a copy of a normal committment form the athletes sign? Generic is fine. We obviously will not use real names or schools. We need this NOW! Can anyone help?

Thanks for the help and the jokes were ignored.
Meydel, Brian Dennehy (Remember him in Rambo-First Blood?) plays the Coach in this movie and I'm not sure in your present condition you could stand up to his grueling practice sessions.

Arkpony, I am available if a 'rich alum' character is needed. I can see me paying off a kids father (played by mrydel) and late in the movie appearing before a judge (hum, who to play that part).
