These included University personnel. The question is whether that also is lack of institutional control? On the other hand Baylor got hit hard after the Dodson incident, but the school was already on probation in other sports, and once the incident occurred, the NCAA found out that Bliss was still Bliss and had numerous rules violations and his handling of the murder itself was horrid, but the murder was not the cause of the NCAA rules violation.
1) While the people at UVA probably could have "done more" to protect the girlfriend, the same can be said about her friends and/or the authority. Ultimately, when something like that happens, there is always 2nd guessing. Bottom line, it appears everyone made "honest" mistakes.
2) Baylor - I would love to blame the university, but the blame for the handling of the murder falls completely on Bliss. The punishment by the NCAA seemed about right given what happened.
3) Perv State U - by 2001, they knew that there was a child rapist using their facilities to rape children and access to their program to recruit victims. For years, they saw Sandusky on campus and at games. For years, they knew he was raping kids. All of them kept quiet b/c they wanted to protect the reputation of Joe Pervterno and his program. The program has to pay a heavy price.
